Frequently Asked Questions

  • Start with scheduling a FREE 15 minute discovery call. This will take you to an online appointment calendar so you can book a time that is convenient for you. Then you will receive a confirmation email with my pricing and discovery call appointment details. After the call, you will receive an email with instructions on how to book your initial consultation and any packages.

  • No. Currently I only accept self pay with a major credit card or HSA/FSA. At your request I am happy to furnish you with a “super bill” which is insurance lingo for an itemized claim that you can submit directly to your insurance for reimbursement on any medical nutrition therapy clinic visit types including nutrition consultations or 1:1 follow-up counseling sessions. Patients accept responsibility for the full amount due regardless of their insurance plan’s reimbursement rate for medical nutrition therapy.

  • I am considered out of network with insurance carriers and so the super bill is provided to patients who are seeking to file a claim for reimbursement. It contains my business EIN and my NPI number along with medical codes that are all required by insurance carriers to process your claim.

    It is always the patient’s responsibility to determine what coverage their policy has for medical nutrition therapy, what the out of network rates are for those services, and any relevant out of network deductibles and coinsurance rates that apply to your policy.

    The usual process is to visit your insurance carrier’s website to find instructions on how to submit a claim and then fill out the required information and attach the super bill.

  • All appointments are via virtual (or phone) format only. If you plan to send in the super bill claim to your insurance, I suggest you choose virtual (face-to-face) format. All virtual appointments will be conducted on a secure, HIPAA compliant platform.

  • All services are paid in full at the time of booking. Please contact me at least 48 hours before your appointment to cancel or reschedule without incurring a fee. If you cancel within 48 hours of your appointment or no show, a $99 cancellation fee will be charged to the card on file. If you choose to cancel your package after your initial appointment, you must contact me by email within 10 days and I will issue you store credit for up to one year from the date I receive the cancellation notice. The Jolley Dietitian does not offer refunds.

  • A surgeon or physician’s referral is not necessary to book services at The Jolley Dietitian. However, if you plan to submit a super bill claim to your insurance, I will need a referral from your surgeon or physician stating your medical diagnoses.

    • Check your insurance coverage for nutrition benefits (then you can ask me for a super bill) and/or your balance on a HSA/FSA account.

    • If you are a pre-surgical patient and your surgeon provided you with patient education materials, please review those and have them handy for the nutrition consultation (pre-surgery patients only) and come ready with questions. The purpose of the consultation visit is primarily to understand you better and to set up a plan for you to establish healthy bariatric lifestyle habits before surgery. Hence there is limited time to cover the education material in depth during that time. The KICKSTART program includes monthly live group Q&A calls to help you with pre-surgical diet education and post-operative diet advancement instructions.

    • When filling out the online health questionnaire and food journal, gather your medications and supplements, and write down some of the typical meals that you eat and beverages that you drink.

  • I will promptly fax any requested consultation or progress notes to your surgeon within two business days. You must have a signed medical records release form on file in order for me to share protected health information with anyone else.

  • Registered dietitians are regulated by licensing boards in most states. I am currently licensed in Washington State and Texas.

    I reserve the right to decline any patient who resides outside of the state(s) in which I am legally licensed to practice in, or for any reason for which I determine that there may exist a conflict of interest. Patients will not be charged if I cancel the consultation for these reasons.

  • Patients enjoy the group sessions because they get peer support AND professional advice from their dietitian. Patients are encouraged to attend each month as part of their program. If group isn’t your thing, then consider booking a 1:1 counseling session a la carte to supplement your package.

  • No. At this time, I do not provide medical weight loss services. Bariatric surgery patients who have undergone sleeve gastrectomy, RNY gastric bypass, duodenal switch, SADI, lap band, endoscopic weight loss procedures, or bariatric revision surgery are my only clientele at this time.

  • YES! I support all bariatric surgery patients to continue to keep a life long relationship with their surgeon’s practice. This is critical to your success and health as your body adapts to surgery over time. I respect and value the importance of your surgeon’s team. The Jolley Dietitian may serve as either supplementary support, or primary support if you are no longer connected to your surgeon’s practice.

    Registered dietitians are NOT medical providers. Seek medical care from your PCP or bariatric surgeon.

  • After booking a discovery call, you will receive an email with an invitation to create an account with Practice Better, an electronic health application that I have partnered with.

    Returning patients: login to your portal account either via web or by downloading the app to your mobile device.
